You are a dedicated and qualified Social Worker eager to make an impact within the Adult Social Care sector, particularly focused on older people, adults with disabilities, and vulnerable adults. You possess in-depth knowledge of social care, along with the legislative frameworks that underpin the practice, and have experience communicating effectively with service users and their families.
What the Job Involves
This role within the East Locality Team involves conducting Care Act Assessments, developing Support Plans, and performing Reviews. You will also engage in CHC Assessments, work with Asylum/NRPF cases, manage transitions, and participate in safeguarding activities. Tasks require you to work flexibly, including face-to-face visits/assessments, remote work from home, and office attendance as needed.
