You have a strong background in care management and have experience collaborating within multidisciplinary teams. What the job involves
As a Mental Health Social Worker in a community setting, you will provide comprehensive assessment, care management, and social work services to adults with mental health problems and their carers. You will be a part of the Barnet, Enfield, and Haringey NHS Mental Health Trust's multidisciplinary Community Mental Health Teams. Your responsibilities will include coordinating care plans, managing service user needs, being part of duty rotas, and undertaking safeguarding adult inquiries. Further, you will use information management systems for accurate documentation and comply with local, council, and mental health trust policies.
