Who You Are
You are an organized and empathetic professional with proven experience in social care, particularly working with adults with psychosis and complex needs. You are adept at managing your time effectively and prioritizing workload. You possess strong interpersonal skills that enable you to engage with service users from diverse backgrounds and work collaboratively within a multidisciplinary team. A thorough understanding of relevant legislation, policy, and procedures, including Safeguarding, the Mental Health Act, the Care Act, and the Mental Capacity Act, complements your practical experience.
What the Job Involves
This role involves engaging in social care activities, focusing on creating and implementing co-produced care and support plans with residents. Duties include risk assessment and management, safeguarding, and conducting holistic assessments of mental health and social care needs. You will also be responsible for carrying out Care Act assessments, and reviews, working closely with carers and support networks, and partnering with both statutory and voluntary organizations. Additionally, the role requires liaising with inpatient units, maintaining accurate records, and report writing.
