You are an experienced professional with a background in working with adults who have psychosis and complex needs. You possess a deep understanding of relevant legislation, policies, and procedures including the Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act. You are organized, capable of managing your time effectively, and prioritizing your workload. Your strong interpersonal skills enable you to engage effectively with service users from diverse backgrounds. You thrive in a teamwork environment and are effective in collaborating with a range of professionals.
What the Job Involves
The role involves coordinating care for complex cases under the Care Programme Approach (CPA). You will develop co-produced care plans alongside service users, conduct risk assessments and management, and ensure safeguarding. You will carry out holistic assessments of mental health and social care needs, conduct Care Act assessments, and work closely with carers and wider support networks. The job requires partnership working with statutory and voluntary organizations, liaising with inpatient units, and maintaining accurate records and report writing.
