You have experience working within a multidisciplinary team, with a focus on care management, and possess excellent written and verbal communication skills for engaging with service users, carers, and other professionals.
What the Job Involves
The role involves providing assessment, care management, and social work services to adults with mental health problems and their carers. You will collaborate with a multidisciplinary team as part of the Barnet, Enfield, and Haringey NHS Mental Health Trust's Community Mental Health Teams. Your responsibilities include coordinating care plans, undertaking safeguarding inquiries, utilizing information management systems, and participating in training as required. You will work in compliance with relevant legislation, policies, and guidelines while maintaining a focus on equal opportunities and health and safety standards.
