Who You Are
You are a dedicated professional with a Diploma in Social Work (DipSW), Certificate of Qualification in Social Work (CQSW) or equivalent, and are registered with the Health and Care Professions Council (HCPC). You possess significant experience and knowledge of mental health work within the statutory framework and have experience in multidisciplinary teamwork and care management. Excellent written, verbal communication, and engagement skills are crucial. Being an Approved Mental Health Professional (AMHP) and having a driving license are desirable.
What the Job Involves
You will provide assessment, care management, and social work services to users and carers in accordance with the Council's statutory duties and policies. As part of a multidisciplinary Community Mental Health Team, you will collaborate with other professionals, undertake needs assessments, manage and review care plans, and act as a care coordinator. Your responsibilities include complying with the Council's financial and recording policies, participating in safeguarding adult inquiries and duty rotas, receiving professional supervision, and staying updated on current legislation and policies. You will also mentor students on placement and may undertake duties as an AMHP.
