Who You Are

You are a compassionate and dedicated practitioner with a passion for supporting children and young people with disabilities and their families. You have a deep understanding of the challenges faced by families in socially deprived areas and have experience working within diverse, multi-cultural communities. You possess excellent communication skills, the ability to build effective relationships, and a commitment to achieving positive outcomes for children with complex needs.

What the job involves

The role of Short Breaks Practitioner involves carrying out high-quality, strengths-based short break assessments and managing a caseload of children with complex needs. You will work closely with families, facilitating a coordinated approach to support, ensuring that educational, health, and social care needs are met. You will align support with EHCP Annual Reviews and encourage family participation in decision-making processes. The role demands innovation in exploring community resources, working collaboratively with agencies, and maintaining meticulous records of interactions and consultations.

Skills

  • Strong knowledge of legislative frameworks (Children Act 1989, Children and Families Act 2014, SEND framework)
  • Experience with assessments and care/transition plan development
  • Proficient in Microsoft Office and IT systems
  • Excellent written and verbal communication
  • Ability to identify and respond to risks in safeguarding
  • Effective relationship-building with children, families, and professionals
  • Resourcefulness, innovation, and creativity
  • Collaborative and able to work integratively with partners
