What the job involves
The role of Short Breaks Practitioner involves carrying out high-quality, strengths-based short break assessments and managing a caseload of children with complex needs. You will work closely with families, facilitating a coordinated approach to support, ensuring that educational, health, and social care needs are met. You will align support with EHCP Annual Reviews and encourage family participation in decision-making processes. The role demands innovation in exploring community resources, working collaboratively with agencies, and maintaining meticulous records of interactions and consultations.
Skills
- Strong knowledge of legislative frameworks (Children Act 1989, Children and Families Act 2014, SEND framework)
- Experience with assessments and care/transition plan development
- Proficient in Microsoft Office and IT systems
- Excellent written and verbal communication
- Ability to identify and respond to risks in safeguarding
- Effective relationship-building with children, families, and professionals
- Resourcefulness, innovation, and creativity
- Collaborative and able to work integratively with partners
