Who You Are
You are a dedicated and professional social worker committed to enhancing the well-being of service users and carers within the Community Mental Health Team at East London NHS Trust. Your focus is on providing high-quality social work services guided by the Care Programme Approach and Care Act.
What the Job Involves
This role involves working both independently and collaboratively within a multidisciplinary team to deliver social work services. You will work with individual service users in hospital settings and community placements, providing advice, assessments, reviews, and other forms of social work assistance. The goal is to improve the outcomes for service users and their carers under the parameters of Social Work England.
Skills
- Strong understanding of the Care Programme Approach and Care Act
- Ability to work collaboratively within a multidisciplinary team
- Proficient in conducting assessments and reviews
- Excellent communication and interpersonal skills
- Knowledge of appropriate legislative requirements
