What the Job Involves
This role requires you to deliver high-quality social work services to adults and carers in accordance with the Council’s duties under the Care Act 2014. You will conduct professional assessments with a focus on delivering outcomes that align with the Adult Social Care Outcomes Framework. Your responsibilities include safeguarding individuals at risk, reviewing care plans, and supporting individuals to maintain their health and independence. You will provide information and advice, signpost services, and work towards building the resilience of service users. Additionally, you will foster a culture of continuous development and ensure adherence to professional standards.
