Who You Are

We are looking for a skilled, motivated, and compassionate Social Worker to join our busy Front Door Service within Adult Social Care. You should be someone who thrives in a fast-paced environment and is driven by the desire to make a significant impact at the point of first contact.

What the Job Involves

In this pivotal role, you will be responsible for responding to new contacts and referrals, working collaboratively with residents, families, carers, and partner organizations to promote wellbeing and independence. Your key responsibilities will include:

• Undertaking complex Care Act assessments, risk assessments, and safeguarding enquiries with a strengths-based and outcome-focused approach.
• Responding to concerns at the front door with professional judgement for proportionate and timely decision-making.
• Applying safeguarding principles and carrying out Mental Capacity Act assessments as needed.
• Providing information, advice, and signposting to community-based options.
• Working closely with multi-agency partners such as health, housing, police, and voluntary sector organizations.
• Maintaining clear, accurate, and high-quality case recordings.
• Contributing to reflective practice and continuous improvement.
