You are a compassionate and skilled Social Worker dedicated to improving the quality of life for individuals with learning disabilities. You have a deep understanding of various social work practices, exceptional knowledge of the Mental Capacity Act, Continuing Health Care, and Safeguarding. With your commitment to evidence-based practice, you contribute positively to team success and high service standards.
What the job involves
This role involves facilitating supported self-assessments to help service users make informed decisions about their services. You will undertake assessments of risk, need, and capacity, working closely with individuals to create personalized support plans. Your work will require a collaborative approach with other professionals to meet the needs of service users, holding a complex caseload, and ensuring high data quality and financial competence. You will be involved in safeguarding, complaints response, and practice enhancement both individually and as part of a team.
Skills
- Excellent knowledge of Social Work practices
- Understanding of the Mental Capacity Act
- Experience in Safeguarding and Continuing Health Care
- Ability to assess risk, need, and capacity
- Financial competency in support planning
- Strong liaison and communication skills
