What the Job Involves
As a specialist SEN and Disability Outreach Worker within Enfield's Service for Disabled Children, your role involves building effective partnerships with parents, carers, professionals, and service providers. You will support children and young people along with their families to access effective short breaks and community activities, provide advice and guidance, conduct short break assessments, and ensure the right support is provided. Additionally, you will collaborate with the SEND Transition Manager to facilitate smooth transition pathways, ensure the views of children and young people inform service development, and undertake the lead professional role when required. You will work closely with Social Workers to ensure progress on support plans.
