What the Job Involves

This role involves managing an allocated caseload, completing comprehensive assessments, and supporting individuals needing care and their families. You will collaborate with other professionals and services to design and implement care and support plans, monitor and review cases, and ensure that decisions are made based on the needs of the individuals. Additionally, you participate in a duty system, responding effectively to emergency cases, conducting crisis assessments, and identifying the necessary resources to mitigate risks.
