What the Job Involves
This role involves supporting the delivery of health and social care services for adults and carers. You will work as part of a multidisciplinary team, conducting assessments, support planning, and providing guidance for adults with complex needs. You will engage in safeguarding processes, manage cases, and mentor newly qualified social workers. Responsibilities include coordinating with external services, participating in community initiatives, and ensuring compliance with relevant policies and legal frameworks. The role demands effective communication, participation in strategy meetings, and prioritization of duties in response to service needs.
